Qualifications

• Current Physical Therapy licensure in state of practice.
• Reliable transportation or access to a vehicle.
• Ability to function in diverse home environments.

Responsibilities

• Evaluate patient's functional status and PT needs; assist physician in plan of care.
• Provide PT services within scope and plan of care, coordinating with the care team.
• Observe and report patient responses to treatment and changes in condition.
• Treat patients to relieve pain, restore function, and maintain maximum functioning.
• Instruct patient and family regarding plan of care, limitations, and progress toward goals.
• Visit patients per Plan of Treatment and document progress notes.
• Complete evaluations and develop PT plan of care per state guidelines.
• Report outcomes of evaluation, goals, and anticipated frequency of care.
