Job summary

CenterWell Home Health is seeking a Therapist (OTA) to deliver home-based rehabilitation services, working under the supervision of a licensed Occupational Therapist. You will help patients regain strength, mobility and independence in their homes, implementing individualized plans of treatment and coordinating with the care team.

The role requires OTA licensure, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

Current OTA licensure and CPR certification.
Valid driver's license and reliable transportation.
Minimum six months occupational therapist assistant experience.
Home Health experience preferred.

Responsibilities

Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
Assist in the implementation of vocational/education programs and activities designed to restore, reinforce, and enhance task performances and independence.
Design/adapt equipment and working or living environments to improve function and independence.
Provide therapeutic treatment and instruction to patients under the direction of the OT and per physician orders.
Report information to OT/Clinical supervisor and document observations in patient records, assist with progress reports and keep required documentation.
Instruct patients and family members regarding home programs and use of adaptive equipment.
Participate in care coordination and discharge planning activities and support the interdisciplinary team.
Design community reintegration activities to assist the client in physical reconditioning and psychological adjustment.
