Job summary

CenterWell Home Health seeks a Home Health Occupational Therapist Assistant to provide therapy services under supervision in patients' homes. You will implement treatment plans, assist in rehab activities, and adapt equipment to restore independence and functional capacity.

The role requires OTA licensure, CPR certification, and a valid driver’s license with reliable transportation. Prior home health experience is preferred, and you will be part of a multidisciplinary team under the Home Health

Qualifications

Current and unrestricted OTA licensure is required.
Current CPR certification is required.
A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
Assist in implementing vocational/education programs and activities established by the OT to restore, reinforce, and enhance task performances.
Design/adapt equipment and living environments; fabricate devices to assist function and independence where possible.
Provide therapeutic treatment and instruction to patients as directed by the OT and physician orders to improve/restore strength, coordination, range of motion and function.
Report observations to OT and/or Clinical supervisor; document in patient records and prepare progress notes; maintain required documentation.
