Job summary

Best Home Health Providers, Inc. is seeking an Occupational Therapist to support patients in their homes across the Bay Area. The role focuses on helping patients regain independence and safely navigate daily activities.

This per diem position offers visits with compensation ranging from $110 to $140 each, with flexible scheduling and potential for full-time conversion based on performance. Travel to patient homes is required in the Bay Area and beyond.

Qualifications

• Graduate OT with ACOTE-accredited program.
• California OT license active required.
• Minimum 2 years OT clinical experience.
• Current CPR certification (AHA).
• Valid driver's license and insured vehicle for travel to patient homes.

Responsibilities

• Evaluate patients' functional abilities and home environments for safety and needs.
• Develop individualized OT treatment plans and communicate with physicians and care team.
• Teach and retrain patients in daily living activities impacted by illness or injury.
• Recommend adaptive equipment and assistive devices.
• Maintain accurate clinical records and report patient changes promptly.
• Coordinate with care team for patient-centered home health care.
