Job summary

YoloCares is seeking a Health Equity Lead Care Manager to help individuals navigate healthcare and social services, provide care management, education, advocacy, outreach, and resource navigation. This lead-level role independently manages an assigned caseload and supports clients with complex needs.

Based in Davis, CA, the regular schedule is Monday–Friday, 8:30 a.m.–5:00 p.m. Travel to client and partner locations is required as needed.

Qualifications

• High school diploma or equivalent required.
• Experience in healthcare, community health, care coordination, or related field preferred.
• Strong communication, relationship-building, organization, and problem-solving skills requested.

Responsibilities

• Build trusting relationships with clients and families; learn goals, strengths, needs, and barriers to care.
• Manage an assigned caseload; provide ongoing care management, education, coordination, and advocacy.
• Help clients navigate healthcare systems; schedule appointments; access benefits; connect with services.
• Coordinate with healthcare providers and community partners to promote continuity of care.
• Conduct outreach and engagement with ECM-eligible and enrolled members.
• Identify social drivers of health affecting access and promote health equity.
