Qualifications

• High school diploma or AA with one year direct care experience, or Bachelor's with one year experience.
• Two years direct care experience preferred.
• Bilingual capacity preferred.
• Must have own vehicle and valid insurance; travel between locations required.

Responsibilities

• Lead care management for Medi-Cal members within CalAIM; coordinate across health and housing services.
• Help Members learn self-management skills via individualized care planning.
• Travel locally; work in community and home environments.
