Vynca Inc. seeks a Lead Care Manager to serve as the primary contact for clients, coordinating with doctors, social services, and other providers to align on care plans. The role emphasizes field and remote care management across the Los Angeles area with travel up to five days a week.

Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations
• Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time, in field and remote with possible evenings/weekends
• Working knowledge of government and community resources related to social determinants of health
• Clean driving record, valid driver's license, and reliable transportation
• Excellent oral and written communication skills
• Positive interpersonal skills
• General computer skills and working knowledge of Google Workspace, MS Office and the internet
• Bilingual English/Spanish, highly preferred

Responsibilities

• Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referrals to community services
• Oversees the development of client care plans and goal settings
• Provide services where the member resides, seeks care, or finds most accessible (office, telehealth, field)
• Connect clients to other social services and supports
• Advocate on behalf of the client with health care professionals
• Utilize evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care
• Conduct outreach and engagement to promote linkage to the ECM program and log activity in CRM
• Evaluate client progress and update SMART goals
• Provide mental health promotion
• Arrange transportation (e.g., ACCESS)
• Complete all documentation including outcome measures within care plans timelines
• Maintain up-to-date patient health records in EMR and other systems
• Complete monthly reporting for program compliance
• Attend training as assigned
