Job summary

Vynca seeks a Lead Care Manager to join our Enhanced Care Management team in Humboldt County. This hybrid role coordinates care across providers, arranging services and guiding care plans to promote wellness and independence.

Requirements include 2+ years in care management, strong communication, and driving ability. English/Spanish bilingual preferred. Travel up to five days per week; residency within 20 miles of the territory is expected.

Qualifications

• 2+ years experience as a care manager or related role.
• Available to work Monday–Friday 8:30am–5:00pm Pacific Time, with possible evenings/weekends.
• Knowledge of government and community resources related to social determinants of health.
• Clean driving record and valid driver’s license.
• Excellent oral and written communication skills.
• Positive interpersonal skills.
• General computer skills with Google Workspace, MS Office and internet.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess member needs across physical and mental health, SUD, memory, housing, and social supports.
• Oversee development of client care plans and goals.
• Provide services where members reside, including office, telehealth, or field.
• Connect clients to social services and supports.
• Advocate with health care professionals for clients.
• Apply evidence-based practices (Motivational Interviewing, Harm Reduction, Trauma-Informed Care).
• Conduct outreach and log activity in the CRM.
