National Youth Advocate Program (NYAP) is seeking a Family Services Coordinator in Newark, OH to advocate for youth and families. The role involves in-home support, transportation, counseling, and connecting families with community resources.

The ideal candidate will have a 4-year degree in social services, be flexible with hours, and willing to travel. Responsibilities include coordinating services, maintaining documentation, and participating in supervision and quality improvement processes.

Qualifications

• Bachelor's Degree in Social Work or comparable Human Services field from an accredited institution.

Responsibilities

• Provide in-home support, transportation, parenting education, counseling, and advocacy.
• Travel daily to deliver community-based services per requirements.
• Complete required documentation and trainings promptly.
• Coordinate services across mental health, medical, educational, and social services.
• Maintain strong relationships with foster families and families of origin.
• Support family reunification and stability per case plan.
• Provide safety assessments and crisis response as needed.
• Meet productivity thresholds and participate in CQI activities.
• Participate in weekly supervision.
