Job summary

National Youth Advocate Program (NYAP) in South Bend, IN seeks a full-time Family Service Coordinator to support youth and families with advocacy, education, and service coordination. The role includes travel to community-based sites, collaboration with foster families, and ongoing documentation and trainings.

Minimum qualifications include a Bachelor's in Social Work or related field and flexible availability.

Qualifications

• Bachelor's Degree in Social Work or comparable Human Services field from an accredited institution.
• 2 years of work experience working in direct service with youth and families strongly preferred.
• Must have flexible availability as needed.

Responsibilities

• Provides in home supportive services, transportation, parenting education, counseling, community resource referral, advocacy, and other social service interventions to youth and family.
• Travels daily to provide community based services to youth and families.
• Completes all required documentation and trainings in a timely manner.
• Coordinates and monitors services for the youth and family in the community, including mental health, medical, educational, psychological, vocational, and social services.
• Reviews and updates youth-based plans to ensure progress toward goals.
