National Youth Advocate Program (NYAP) seeks a Family Service Coordinator in Terre Haute, IN. You will work under supervision to advocate for and support children and families, delivering in-home services, transportation, and counseling to help them live safely in the community.

Qualifications

• Bachelor's Degree in Social Work or comparable Human Services field from an accredited institution.
• Must have flexible availability as needed.
• Must have a valid driver’s license, reliable transportation, automotive insurance, and a good driving record.
• Must have car insurance requirements of 100,000/300,000 bodily injury insurance.
• Willingness to work flexible and non-traditional hours in the service of families of origin.

Responsibilities

• Actively supports, represents, and extends the mission, vision, and values of the organization.
• Provides in home supportive services, transportation, parenting education, counseling, and advocacy.
• Travels daily to provide community based services to youth and families.
• Completes all required documentation and trainings in a timely manner.
• Coordinates and monitors services for youth and family across multiple systems.
• Reviews and modifies youth plans to ensure progress and success.
• Maintains strong relationships with foster parents and families of origin.
• Assists in family reunification and stabilization efforts as per case plan.
• Provides safety assessment, crisis response, and behavioral stabilization as required.
• Achieves established productivity thresholds and participates in CQI activities.
