Qualifications

• Masters in Social Work required.

• 0–2 years’ related experience.

Responsibilities

• Assess patients' psychosocial status and develop care plans with the interdisciplinary team.
• Provide supportive counseling to patients and families.
• Educate patients about ESRD, treatment options, and transplant referrals.
• Assist with advance directive discussions and access to resources.
• Coordinate referrals to community services and vocational rehabilitation.
• Support patient education on treatment modalities and rights/responsibilities.
• Document interventions and maintain up-to-date patient records.
• Participate in quality improvement activities and team huddles.
