Job summary

Paycom invites applications for a Camp Rehousing Case Manager to support OKC's encampment rehousing initiative. You will manage a caseload of 20 individuals, connect street homeless clients to housing resources, and coordinate with Street Outreach, Landlord Engagement, and Closure Maintenance teams to sustain engagement through housing transitions.

Qualifications

• Undergraduate degree in social work or behavioral science, preferred.
• Two+ years of previous experience in adult case management.
• Computer literate; experienced with Microsoft Office (Word, Excel), Outlook, Internet.

Responsibilities

• Coordinate with Street Outreach, Landlord Engagement, and Closure Maintenance teams to assist clients in encampments and maintain contact for a year after housing.
• Complete HMIS program enrollments and documentation.
• Assist clients with unit and voucher applications.
• Assist with move-ins.
• Provide one year of weekly home-based case management, including intake, long-term housing stabilization planning, budget planning, opportunities to increase income through employment or disability assistance, connecting to behavioral health and physical healthcare services, and eviction prevention.
• Document case interactions in HMIS with detail.
• Attend daily encampment huddles and monthly stabilization workgroups.
• Liaise with service providers to increase stabilization.
• Act as point of contact for Coordinated Entry staff and partners.
• Follow updated CoC Standards of Care; participate in CoC workgroups.
• Demonstrate flexibility, kindness; provide transportation as needed.
• Perform other related duties.
