Job summary

MECA Therapies, LLC in Clovis, New Mexico, is seeking a compassionate Family Service Coordinator (FSC) to support families with children birth to three through Early Intervention. The role centers on coordinating services, guiding IFSP development, and connecting families to resources in home and community settings.

You will collaborate with therapists, healthcare providers, and community partners to ensure timely, family-centered services, with emphasis on confidentiality, eligibility

Qualifications

  • Bachelor's degree in Social Work, Counseling, Psychology, Nursing, Special Education, or related field.
  • Two years of direct experience serving individuals with disabilities and/or families may substitute for a degree.
  • Ability to complete required FSC training within one year.
  • Strong organizational and record-keeping skills.
  • Excellent verbal and written communication skills.
  • HIPAA confidentiality and compliance.
  • Proficiency with Windows and electronic documentation systems.
  • Valid driver's license and reliable transportation.

Responsibilities

  • Serve as the primary contact for families enrolled in Early Intervention services.
  • Explain EI process, family rights, safeguards, and resources.
  • Gather information on family concerns, priorities, resources, and outcomes.
  • Coordinate evaluations, eligibility, and IFSP service planning.
  • Assist families in developing and preparing IFSPs.
  • Coordinate and monitor EI services identified on the IFSP.
  • Connect families with community resources and medical providers.
  • Facilitate TTCMs, IFSP meetings, and reviews.
  • Ensure services are timely and aligned with family priorities.
  • Support families in development across cognitive, communication, physical/motor, social-emotional, and adaptive domains.
  • Assist with insurance navigation and documentation.
  • Complete transition planning and support transition conferences.
  • Conduct initial and ongoing family visits.
  • Maintain strong relationships and family-centered practices.
