Job summary

Default Brand seeks an Early Intervention Services Coordinator (Case Management) to serve Riverside South County. You will coordinate assessments for infants eligible under IDEA Part C, educate families on developmental strategies, and arrange timely intervention services.

The role covers a wide geographic area with mileage reimbursements for travel. You will develop and implement IFSPs, perform required case management tasks, and liaise with referring professionals.

Qualifications

• Master’s Degree in social work, psychology, public health nursing, teaching, early childhood development, or a related field.
• BA or BS with one year of related experience (case management, IDD, social work, special education, or early childhood development).
• Ability to be flexible and adaptable with changing priorities and assignments.
• Maintain confidentiality and follow directions.
• Strong verbal and written communication skills.
• Must have a valid driver's license and reliable transportation; insured vehicle required.
• May require bilingual abilities.

Responsibilities

• Coordinate the assessment of infants eligible for IDEA Part C services.
• Educate parents on developmental strategies and IFSP planning.
• Arrange for appropriate services and interventions; monitor progress.
• Maintain contact with families and protect consumers’ rights and dignity.
• Lead annual reviews, Baby CDER, CDER, and IFSP updates.
• Represent the agency at interagency meetings and case conferences.
• Complete administrative tasks and required documentation on time.
• Ensure transition planning for children aging out of Early Start.
