Job summary

The role includes coordinating evaluations, IFSP/FSP development, family education on rights, monitoring services, documentation, and collaboration with families, providers, and agencies, with community-based travel required.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Early Childhood Education, Special Education or related field.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities.
• Experience with family-centered supports and coordination preferred.

Responsibilities

• Provide primary contact for families and coordinate access to services.
• Coordinate evaluations, IFSP/FSP development and reviews.
• Educate families on rights and safeguards and promote empowerment.
• Monitor service delivery and coordinate across service systems.
• Maintain accurate documentation and complete service notes.
• Collaborate with families, providers, and agencies.
