Job summary

Default Brand in Riverside Central area is seeking an Early Intervention Services Coordinator (Case Management) to serve families with infants eligible under IDEA Part C. You will coordinate assessments, educate parents on developmental strategies, arrange services, document IFSPs, and perform case management.

The role requires Master’s or BA with experience in relevant field, bilingual preferred, and will involve travel to Hemet, Homeland, and surrounding areas with mileage reimbursement.

Qualifications

• Master’s Degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field.
• BA or BS degree from an accredited college or university and one year of experience, including case management, in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field.

Responsibilities

• Assume daily case management responsibilities.
• Understand California’s regulations for the Early Start Program, and Lanterman Act.
• Maintain contact with consumers and families; assure rights and dignity.
• Coordinate annual review process including Baby CDER, CDER, Annual Review and IFSP.
• Provide advocacy services; perform crisis intervention and document activities.
• Arrange for placement of consumers in infant educational programs or training programs.
• Perform assessments and developmental testing such as First Look, Denver II, REEL III, Music 2 My Ears, M-Chat, Day-C.
