Job summary

Easterseals Hawaii is seeking a Care Coordinator (EI) to support families with young children showing developmental delays. The role emphasizes partnership with families, providers, and a multidisciplinary team to promote growth and meet treatment goals.

The position requires coordinating monitoring and services, documenting care plans, and ensuring adherence to program guidelines while maintaining in-person presence at the office 4–5 days per week.

Qualifications

• Bachelor’s degree in Social Work, Social Service, Education or equivalent from an accredited college or university program.
• Active LSW or LCSW license required.
• One (1) year minimum experience in professional social work is highly preferred.
• Experience working with children with special needs in the field of early intervention is highly preferred.

Responsibilities

• Coordinate child development monitoring and intervention services to children ages 0-3 with developmental delay or disability.
• Model strategies and provide education to families/care providers to promote child development.
• Collaborate with the team on child outcomes and services in the Individualized Family Service Plan (IFSP).
• Collaborate with Hawaii DoH Early Intervention Section and meet funder guidelines, timelines, and caseload reporting.
• Meet productivity requirements for assigned caseload and provide intervention strategies and resources.
• Facilitate weekly Family Support Team meetings under supervision.
• Schedule home visits or telepractice visits based on family preference for service delivery.
• Coordinate family visits with other providers as needed.
• Assess, evaluate, and consult on child development progress using evidence-based tools.
• Complete billing and service plan documentation within guidelines.
