Job summary

Easterseals Hawaii is seeking a Care Coordinator (EI) to support families of children birth to 3 with developmental delays. You will monitor progress, coordinate services, educate families, and ensure program guidelines are met.

This full-time role requires a Bachelor's in social work or related field, an active LSW/LCSW, and at least one year of experience. You will collaborate with teams, document thoroughly, and report to the EI Program Manager.

Qualifications

• Bachelor’s degree in Social Work, Social Service, Education or equivalent from an accredited college or university program.
• Active LSW or LCSW license required.
• One year minimum experience in professional social work is highly preferred.
• Experience working with children with special needs in early intervention is highly preferred.

Responsibilities

• Coordinates child development monitoring and intervention services for 0–3 year olds with or at risk of developmental delay.
• Models strategies and educates families to promote child development.
• Collaborates with the team to monitor outcomes on the IFSP.
• Meets funder guidelines, timelines, and caseload reporting.
• Facilitates weekly Family Support Team meetings under program management.
