Job summary

Easterseals Hawaii is seeking a Care Coordinator (EI) to support families of children 0-3 with developmental delays or risk. You will coordinate services, educate caregivers, and monitor progress under the Early Intervention framework.

You will partner with a multidisciplinary team, schedule home or telepractice visits, complete required documentation, and help families access community resources to promote growth in natural environments.

Qualifications

• Bachelor’s degree in Social Work, Social Service, Education or equivalent.
• Active LSW or LCSW license required.
• One year of professional social work experience preferred.
• Experience with children with special needs in early intervention preferred.
• Ability to work with diverse families and communities.

Responsibilities

• Coordinate child development monitoring and intervention services for 0-3 years.
• Educate parents/caregivers and promote child development.
• Collaborate to monitor outcomes on the IFSP and ensure funder guidelines.
• Maintain productivity and complete necessary service-plan documentation.
• Attend Family Support Team meetings and schedule home or telepractice visits.
• Communicate effectively with families and multidisciplinary team.
