Easterseals Hawaii is hiring a Care Coordinator (EI) to support children from birth to age 3 with developmental delays or at risk of delay. The role emphasizes a family-centered approach, collaboration with the Early Intervention team, and adherence to program guidelines.

The caregiver will conduct home or telepractice visits, collaborate with families to set goals, and coordinate services to maximize learning potential while staying current with best practices in early intervention.

Competenze

• Bachelor’s degree in Social Work, Social Service, Education or equivalent.
• Active LSW or LCSW license required.
• One year minimum experience in professional social work preferred.
• Experience working with children with special needs in the field of early intervention is highly preferred.

Mansioni

• Coordinates child development monitoring and intervention services for 0–3 yrs with developmental delay or disability.
• Educates and supports families to promote their child’s development.
• Monitors child outcomes on the Individualized Family Service Plan (IFSP).
• Coordinates with Hawaii DOH EI section and meets funder guidelines and timelines.
• Maintains caseload productivity and provides intervention strategies and family resources.
• Facilitates weekly Family Support Team meetings under supervision.
• Schedules home visits or telepractice per family preference for natural environment activities.
• Completes service plan documentation and billing within guidelines.
• Provides referrals and resources to families as needed.
• Keeps current on research and developments in special education.
