Job summary

Alden Village in Bloomingdale, IL is seeking a qualified professional to coordinate Active Treatment Programs for residents with developmental and intellectual disabilities, ensuring compliance with federal, state, and local standards and facility policies.

The role requires a bachelor’s degree in a Human Services field, at least 1 year of direct experience, CPR certification within 90 days, strong English communication, independent decision-making, tact, patience, and a collaborative approach

Qualifications

• Bachelor’s degree in a Human Services field.
• At least 1 year of direct experience with intellectual/developmental disabilities.
• CPR certification current or within 90 days of employment.
• Must read, write and speak English clearly.
• Ability to make independent decisions when needed.
• Ability to work harmoniously with residents, families, staff and others.
• Patience, tact, a cheerful disposition and enthusiasm.

Responsibilities

• Coordinate, integrate, and monitor Active Treatment Programs for assigned residents.
• Attend and actively participate in IDT conferences.
• Develop written care plans identifying Active Treatment needs for residents.
• Maintain IPP and active treatment goals monthly.
• Ensure staff follow policies and report concerns to leadership.
• Plan and monitor the 24-hour schedule for assigned clients.
• Liaise between facility, residents' families/guardians, and school personnel.
• Support staff through education and in-service programs.
