Job summary

The Alden Network is seeking a dedicated professional to integrate, coordinate, and monitor Active Treatment Programs for individuals with developmental and intellectual disabilities. The role requires collaboration with nursing, family and school personnel, and active participation in IDT conferences to optimize resident care.

You will develop individual plans, monitor progress, and ensure timely IPP updates while maintaining a positive department atmosphere and ongoing staff development.

Qualifications

• Bachelor’s degree in Human Services field (e.g., Psychology, Sociology, Education Counseling, etc.).
• At least 1 year of experience with intellectual/developmental disabilities.
• CPR certification or certification within 90 days of employment.
• Proficiency in reading, writing, and speaking English.
• Ability to make independent decisions when warranted.
• Ability to work tactfully with residents, families, staff, and agencies.
• Patience, tact, and enthusiasm.

Responsibilities

• Coordinate and monitor Active Treatment Programs for assigned caseload.
• Attend IPT/IDT conferences and participate actively.
• Develop written plans of care for each resident.
• Communicate concerns to administration.
• Review and monitor daily schedules, logs, and progress notes.
• Maintain warm, positive environment in the department.
• Plan and participate in in-service education and orientation programs.
• Assist with equipment planning and supply needs.
