The Familiar Face Case Manager is a behavioral health technician or behavioral health p rofessional specializing in the care coordination and discharge planning of individuals utilizing crisis services at disproportionately high rates. The patient population is determined by individuals who are some of the highest utilizers of crisis services. The Familiar Face Case Manager’s objective is to help prepare the patient for the most clinically appropriate services, assist the patient in obtaining those services, coordinating care with all involved parties, and reducing barriers to effective treatment. The Familiar Face Case Manager reports to the Clinical Director.
