Job summary

The Familiar Face Case Manager at Community Bridges, Inc. focuses on care coordination and discharge planning for individuals using crisis services who are high utilizers.

The role involves preparing patients for appropriate services, helping them access those services, and coordinating with all parties while reducing barriers to effective treatment. Reporting to the Clinical Director, this position requires dedication to compassionate behavioral health care and offers opportunities for

Qualifications

• High school diploma or GED required; associates degree preferred.
• Minimum of six (6) months of experience working in multiple settings and levels of care (mental health and substance abuse).
• One (1) or more years of experience preferred.
• Minimum of six months of recovery from substance use and/or mental health disorders preferred.
• Current Driver’s License (valid and in good standing).
• 39-month Motor Vehicle Record (MVR).
• Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Policies.

Responsibilities

• Coordinate care and discharge planning for crisis service users.
• Assist patients in obtaining clinically appropriate services.
• Coordinate care with all involved parties and reduce barriers to effective treatment.
• Report to the Clinical Director.
