Job summary

Behavioral Health Network (BHN) in Western Massachusetts seeks a Counselor-Case Manager to join the Transitional Support Services team. You will provide direct counseling, manage a caseload in residential care, and develop individualized treatment plans with the treatment team.

This role emphasizes recovery-oriented approaches, coordination of discharge planning, and connections to housing, healthcare, and community resources.

Qualifications

• Requires high school diploma or GED with preferred associate degree.
• Experience in substance-use treatment environment is required.
• Knowledge of recovery-oriented care and community resources preferred.

Responsibilities

• Provide direct counseling, case management, and recovery support within Transitional Support Services (TSS).
• Manage assigned caseload and meet individuals as per level of care.
• Complete admission and discharge documentation and develop treatment plans and safety plans.
• Coordinate discharge and aftercare planning with referrals to outpatient treatment, housing, and community resources.
• Document participation and progress in the electronic health record.
• Collaborate with multidisciplinary team to ensure quality care.
• Maintain HIPAA and 42 CFR Part 2 confidentiality and complete required trainings.
