Job summary

Behavioral Health Network (BHN) in Western Massachusetts seeks a Counselor-Case Manager to join the Transitional Support Services team. You will provide direct counseling, manage a caseload in residential care, and develop individualized treatment plans with the treatment team.

This role emphasizes recovery-oriented approaches, coordination of discharge planning, and connections to housing, healthcare, and community resources.

Qualifications

• Requires high school diploma or GED with preferred associate degree.
• Experience in substance-use treatment environment is required.
• Knowledge of recovery-oriented care and community resources preferred.

Responsibilities

• Provide direct counseling, case management, and recovery support within Transitional Support Services (TSS).
• Manage assigned caseload and meet individuals as per level of care.
• Complete admission and discharge documentation and develop treatment plans and safety plans.
• Coordinate discharge and aftercare planning with referrals to outpatient treatment, housing, and community resources.
• Document participation and progress in the electronic health record.
• Collaborate with multidisciplinary team to ensure quality care.
• Maintain HIPAA and 42 CFR Part 2 confidentiality and complete required trainings.

What You Will Do

• Provide direct counseling, case management, and recovery support to individuals receiving residential care within Transitional Support Services (TSS).
• Manage an assigned caseload, meet with individuals at the frequency required by their level of care, and build supportive relationships that promote recovery, independence, and community integration.
• Complete admission and discharge documentation and develop individualized assessments, treatment plans, safety plans, and related service documentation with individuals and the treatment team.
• Coordinate discharge and aftercare planning, providing referrals and connections to outpatient treatment, recovery supports, housing, healthcare, benefits, and other community resources.
• Facilitate diverse psychoeducational and family groups and document participation, progress, and services accurately in the electronic health record.
• Participate in multidisciplinary treatment-team meetings and collaborate with the Program Director, Clinical Supervisor, Recovery Specialist Supervisor, and other team members to ensure quality care.
• Monitor program activities and support individuals in following program expectations while using respectful, person-centered, and recovery-oriented approaches.
• Respond appropriately to critical incidents and emergencies, use on-call support when needed, and promptly report safety, maintenance, or environmental concerns.
• Maintain complete, accurate, and timely electronic health records, incident reports, transportation requests, and other program documentation.
• Protect confidentiality under HIPAA and 42 CFR Part 2, follow licensing and funding requirements, complete required training and supervision, support transportation as needed, and perform other assigned duties.

Who You Are

• High school diploma or GED required; associate degree or higher preferred.
• At least one to three years of experience working in a substance-use treatment environment required.
• Knowledge of substance-use disorders, recovery-oriented care, case management, treatment planning, relapse-prevention supports, and community resources.
• Commitment to empowerment, personal growth, human rights, dignity, safety, and community integration for individuals served.
• Two years of sustained recovery preferred for candidates drawing on personal recovery experience.
• Strong interpersonal, group-facilitation, verbal, and written communication skills, including the ability to respond professionally during stressful situations.
• Strong organization, time-management, documentation, and follow-through skills, with the ability to manage multiple priorities and urgent needs.
• Familiarity with internet, email, word-processing, agency communication, and electronic health record systems.
• Valid driver's license required, along with the ability to complete required driving training and support transportation when assigned.
• Ability to work the assigned essential-staff schedule in a residential program and meet the physical requirements of the role, including lifting or carrying up to 25 pounds, with or without reasonable accommodation.

Available Schedules

Full Time: Sunday-Wednesday, most days: 8:30am-5:00pm with 1-2 late days, 11:00am-7:30pm or 12:00pm-8:30pm

Full Time: Sunday-Thursday or Tuesday-Saturday, 11:00am-7:30pm or 12:00pm-8:30pm

Pay Range: $21.50-$24.50/hr
