Health Connect America is seeking a case management professional to deliver high-quality services for children, families and adults. The role emphasizes documentation aligned with payer, state, and COA standards, and collaboration with the Treatment Team.

Responsibilities include weekly or bi-weekly client meetings for up to 12 months, 24/7 on-call support, and coordination with community resources. Mississippi credentialing and ongoing compliance are required.

Formación

• Bachelor’s degree in human services and experience with children/families in case management.
• Provisional credentialing as a PCSS in Mississippi with eligibility to be CSS in Mississippi.

Responsabilidades

• Provide best practice, quality case management; ensure documentation meets payer, state, and COA standards.
• Connect families with community resources and follow-up on referrals.
• Complete documentation within established time frames.
• Meet with clients weekly or bi-weekly for up to 12 months.
• Attend weekly Treatment Team meetings.
• Serve as liaison with other professionals and agencies.
• Provide 24/7 on-call support as per program requirements.
• Ensure compliance with state regulatory bodies.
