Job summary

Health Connect America in Mississippi is seeking a dedicated case manager to deliver quality, evidence-informed services to children, families and adults. You will document per payer and COA standards, coordinate services, and connect clients with community resources, while updating records and collaborating with the treatment team.

Qualifications

• Bachelor’s degree in a human services field (Social Work, Psychology, Sociology, Counseling, or Criminal Justice).
• Provisional PCSS credentials in Mississippi required; eligibility for full CSS credentials in Mississippi.

Responsibilities

• Provide best practice, quality case management services in line with Payer, State, and COA standards.
• Assist in marketing referrals and communicate with clients, families, and professionals.
• Maintain timely documentation and reports; complete a recovery support plan within 30 days of referral.
• Meet with clients weekly or bi-weekly for up to 12 months and participate in weekly Treatment Team meetings.
• Serve as liaison with other professionals and agencies; provide 24/7 on-call support as required.
• Ensure compliance with all state regulatory bodies.

Education

Bachelor’s degree in a human services discipline
