Job summary

Health Connect America in Mississippi seeks a dedicated case management professional to coordinate services for children and families. You will connect clients with community resources, document progress, and collaborate with treatment teams in a home- and community-based setting.

The role requires a Bachelor's degree in a human services field and provisional PCSS credentials in Mississippi, with eligibility for full CSS credentials.

Qualifications

  • Bachelor’s degree in a human services discipline such as Social Work, Psychology, Sociology, Counseling, or Criminal Justice.
  • Must hold provisional PCSS credentials in Mississippi and be eligible for full CSS credentials in the state.
  • Experience in case management or community resource positions with children and families is preferred.

Responsibilities

  • Communicate with clients, families, and agencies to answer questions and address concerns.
  • Connect families with needed community resources and document referrals.
  • Complete documentation within established time frames and maintain files.
  • Meet with clients weekly or bi-weekly for up to 12 months and participate in weekly treatment team meetings.
  • Provide 24/7 on-call support as program requirements.
  • Ensure compliance with all state regulatory bodies and program standards.

Our Brands

Provide best practice, quality case management services, ensure that all documentation is completed per Payer, State, and COA standards and requirements. Assist in marketing new referrals.

  • Communicate with customers, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints/concerns.
  • If no wraparound facilitator is present with MYPAC, CSS will make 2 contacts per week by phone or face to face and document accordingly.
  • Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.
  • Complete documentation within established time frames.
  • Meet with clients weekly or bi‑weekly for up to 12 months. (see above for MYPAC requirements).
  • Attend and participate -weekly with Treatment Team meetings.
  • Serve as a liaison with other professionals and agencies related to the client.
  • Complete a recovery support plan with all individuals within the first 30 days of referral to services.
  • Provide 24/7 on‑call support for clients as per program requirements.
  • Ensure compliance with all state regulatory bodies.
  • A Bachelor’s degree in a human services discipline, such as Social Work, Psychology, Sociology, Counseling, or Criminal Justice, along with relevant experience in case management or community resource positions with children and families.
  • Candidates must hold provisional credentials as a PCSS in Mississippi and be eligible for full CSS credentials in the state.
