Job summary

Qualifications

Responsibilities

• Provide direct care and daily living support to individuals with IDD.
• Assist with personal care, grooming, meals, and other ADLs.
• Implement ISPs and document progress accurately.
• Transport and accompany clients to medical appointments and activities.
• Monitor health conditions per care plan guidelines and administer medications as allowed.
• Foster independence, dignity, and community inclusion.
