Job summary

Support, Inc. is hiring for a Day Services role in Greeley, CO. You will lead meaningful activities for adults with IDD at the center and in the community, helping them grow skills and build independence.

This is a full/part-time, on-site position with varied hours to meet client needs. You’ll work closely with a team to promote inclusion, provide hands-on support, schedule and accompany activities, and document progress.

Qualifications

• Active and engaged in community activities and events.
• Tech-savvy with ability to perform basic computer tasks (emails, documentation).
• Able to pass required background check without issues.
• Have reliable transportation, valid license, and current auto insurance.
• H.S. diploma or GED completed; experience with IDD is a bonus.

Responsibilities

• Promote Community Integration by planning and facilitating activities in community and center.
• Provide Personalized Support with hygiene, meals, and household tasks.
• Provide Transportation and Accompaniment to appointments, errands, and outings.
• Support Medical needs including scheduling/attending appointments and monitoring health.
• Develop life skills through training in communication, problem-solving, and daily living activities.
• Document progress with accurate notes and logs.
• Flexible Scheduling to meet the needs of individuals supported.
