Job summary

Thames Valley Council for Community Action, Inc. seeks a Client Services Navigator-Community Services to perform universal intakes, assessments, referral to TVCCA programs, and application assistance for DSS programs for clients needing general case management.

They collaborate with Energy Assistance, Financial Literacy, and SNAP Outreach staff to deliver comprehensive services to TVCCA’s eligible client base, follow up on referrals, and provide ongoing, outcome-oriented case management.

Qualifications

• Experience providing intake and case management services in a community services setting.
• Strong communication and organizational skills.
• Ability to work collaboratively with cross-functional teams.

Responsibilities

• Perform universal intakes and assessments for clients.
• Provide referrals to TVCCA programs and assist with DSS applications.
• Conduct information and referral services and ensure follow-up on referrals for outcome-oriented case management.
