Job summary

Connecticut Community Care in Connecticut seeks a Program Navigator to coordinate and implement long-term services and support in the community. Under the direction of the Assistant Director you will assist in referral screening, assessments, and activation onto programs, contributing to CCC's mission.

You will work as part of an interdisciplinary team, communicate with clients and referral sources, and help mentor new staff.

Qualifications

• Project management and organizational skills.
• Strong verbal and written communication skills.
• Knowledge of community resources preferred.
• Ability to maintain confidentiality of client and company information.
• Ability to understand and articulate multiple programs.
• Working knowledge of Connexus.

Responsibilities

• Provides Referral Screening for all new potential clients of Public Programs supported by CCC.
• Provides telephonic communication with client and/or the referral source to provide education and program navigation.
• Schedules initial assessments.
• Follows, supports and assists pending client's post-assessment to assist with timely activation onto appropriate program/s; may include home visits.
• Participates in and works effectively as part of an interdisciplinary team.
• Assist in mentoring new staff as needed.
