Job summary

Connecticut Community Care is seeking a Program Navigator under the direction of the Assistant Director to assist in coordinating and implementing long-term services and supports in the community. This role helps individuals connect with essential services to live their best lives at home and stay engaged with their communities.

Qualifications

• High School Diploma required.
• Associate Degree in Health or Human Service preferred.
• Strong verbal and written communication skills.
• Knowledge of community resources preferred.
• Ability to maintain confidentiality of client and company information.

Responsibilities

• Provides Referral Screening for all new potential clients of Public Programs supported by CCC.
• Provides telephonic communication with client and/or the referral source to provide education and program navigation.
• Schedules initial assessments.
• Supports post-assessment activation onto appropriate programs, including home visits as needed.
• Participates in and works effectively as part of an interdisciplinary team.
