Our Addiction Services Division offers a full continuum of care, including residential detox, outpatient mental health and substance use services for adults and children, intensive outpatient programs, and

• The Community Health Navigator supports Hartford residents in accessing and staying engaged with medical, behavioral health, and social services.

Mansioni

• Coordinate care across primary care, behavioral health, and social services.
• Assist clients in navigating appointments, resources, and community supports.
• Support individuals in addiction services contexts across continuum of care.
• Provide outreach and engagement support within Hartford communities.
• Support mobile crisis evaluations and community-based services when appropriate.
• Assist with program navigation, including residential detox and outpatient services.
• Facilitate connections to employment and social rehabilitation resources.
