Job summary

Care New England is seeking a Community Health Worker to join the integrated health team and support patients with complex medical, behavioral health, and social needs through home- and community-based services.

The role emphasizes navigation, care coordination and ongoing case management, with patient-centered outreach and collaboration with primary care to improve health outcomes across Rhode Island communities.

Qualifications

• High School diploma with experience in community-based settings.
• Certification in Community Health Work or working toward certification; must obtain within 12 months.
• Associate's or Bachelor's degree in Social Work, Community/Public Health or related health sciences field is a plus.
• Experience working with primary care providers or in other healthcare settings.
• Experience managing patients' health, navigating systems, and health coaching is preferred.
• Ability to travel to various locations in the state; reliable transportation required.
• Must possess a valid driver’s license, reliable transportation and proof of auto insurance.
• Ability to speak a second language; Spanish preferred.

Responsibilities

• Assist In-Home patients in securing community resources identified through SDOH screening and in-home assessment.
• Act as liaison between patient and community resources.
• Conduct SDOH screening and assess patients' needs to remain in the community.
• Support chronic condition disease management programs for BCBS members.
• Provide navigation, care coordination, and ongoing case management.
• Visit patients in their homes and communities, delivering culturally sensitive health information.
• Facilitate patient decision-making and self-management to improve health outcomes.
• Track patient-related activities and document progress.
• Collaborate with the primary care team in multidisciplinary meetings.
• Engage with community agencies to identify resources and improve population health.
