• Associate degree required; Bachelor's degree preferred.
• 1-3 years' experience in health care, human services, or relevant volunteer work.
• Valid driver's license is required.
• Bilingual Spanish strongly preferred.

Responsabilidades

• Engages individuals during inpatient hospitalization, outpatient appointments and community outreach visits.
• Informs individuals and families about resources and helps navigate access to services.
• Assists clients in accessing health-related services and obtaining a medical home.
• Coaches and assists patients with MyChart sign up and uses care coordination workflows.
• Travels to outreach destinations including patient homes, appointments, and community locations.
• Documents all client interactions in the electronic health record with timely notes.
