Apply Based On Your County
  • By working in the County you live in you will have a better understanding of the need, concerns and resources in the community, which can help you to provide better service to the people you are supporting.
  • Working within the County you live in will ensure that you are able to meet the expectations of the position. For example, our Case Managers are responsible for billing roughly 75% of their worked time and, travel time, while it is worked and paid time, is not a billable activity. Therefore, traveling too far to support the people on your caseload will inhibit your ability to meet the expectations of the position.
  • You will be eligible for mileage reimbursement at the federal rate.
SCI Summary

SCI supports people with disabilities, behavioral challenges, medically complex needs, transitioning youth, the elderly, and other populations using Maryland Department of Health’s Targeted Case Management and Supports Planning work models.

Position Summary

Provide Case Management services to individuals who have an intellectual or developmental disability. Primary objectives of this position include establishing positive working relationships through a solution-based approach, routinely exploring options, areas of interest and preferences for each person served, establishing and monitoring an annual Person-Centered Plan (PCP) that identifies outcomes, outlines required support, and ensures ongoing services and continuous evaluation of service quality. Meet state-mandated requirements for all duties of the Service Coordinator role and ensure that the services provided meet state-mandated standards.

SCI Team Member Expectations

All SCI Team Members are expected to: ensure services follow the organization’s mission, core values and policies; follow HIPAA and confidentiality procedures; collaborate with peers, leadership and support areas; actively participate in meetings and trainings; maintain compliance with federal, state and local employment laws; follow person-centered planning principles; and foster a culture that values diversity.

Essential Duties
  • Advocate on behalf of individuals, provide resource information and educate them to become more self-determined.
  • Provide education to individuals and their families, and on how to coordinate and advocate for services.
  • Encourage individuals to empower themselves and achieve optimal independence.
  • Educate individuals on self-determination principles.
  • Collect information and documentation related to eligibility for DDA services and recommend eligibility and priority to DDA.
  • Meet with individuals and/or family to obtain information and relevant documentation.
  • Complete a Comprehensive Assessment (CA) and plan for, update and develop Person Centered Plans (PCPs).
  • Engage in person-centered planning process, develop and write PCPs in collaboration with the individual, conduct PCP preparations and write the Self-Directed Plan.
  • Research, identify, refer, and coordinate resources and services to achieve PCP outcomes; share resources with team members.
  • Travel to meet with support staff, provide information, make referrals, and assist individuals; help with transitions.
  • Complete required paperwork, follow up for waivers, identify services for individuals in state facilities and those with forensic involvement, monitor PCPs and conduct follow-up activities.
  • Conduct on-site visits, timely contacts, obtain information on progress, submit Recertification of Need (RECON), identify new medical and health services.
  • Submit recommendations for new DDA priority category, complete Agency Investigation Report (AIR), reapply for necessary programs or services.
  • Communicate with providers regarding resolution of concerns, oversee documentation for medical assistance, adhere to SCI policies and procedures.
  • Meet performance utilization benchmark, maintain HIPAA records, and utilize tracking and time management tools.

Education Requirements
  • A bachelor's degree from an accredited program in a human services field; OR
  • A bachelor’s degree in a non-human services field, with two years of experience in a human services field.
Experience Preferred

Experience working with people with developmental disabilities.

Physical Demands

Generally presents a standard office environment. Must remain stationary >95% of the time, operate computer equipment, and occasionally move office equipment weighing 10-25 pounds. Vision requirements include close vision. Ability to operate a motor vehicle. Frequent communication with others required.
