- Experience with developmental disabilities preferred.
- Willingness to travel to meet with individuals and teams.
Responsibilities
- Advocate for individuals, provide resources, and educate on self-determination.
- Collect eligibility information and help plan PCPs for DDA services.
- Plan and update Person Centered Plans documenting service needs.
- Identify resources, make referrals, and coordinate services across providers.
- Monitor PCPs and conduct follow-up activities to ensure quality.
- Adhere to SCI policies, HIPAA, and participate in trainings.
