LifeStream is seeking a Case Manager I to advocate for clients, link them to services, and monitor progress toward independence. You will work closely with clients, families, LifeStream staff, and community agencies to address needs and support sustainable outcomes.

The role emphasizes assessment, service planning, resource coordination, and ongoing documentation, with opportunities for crisis support and professional development within a supportive team environment.

Qualifications

• Bachelor’s degree in human services or related field required.
• At least 1 year of case management or related experience.
• Ability to assess needs and develop service plans.
• Experience with diverse populations and community resources.

Responsibilities

• Conduct thorough assessments to identify client needs and goals.
• Develop, implement, and monitor individualized service plans.
• Coordinate access to community resources and supports.
• Advocate for clients within healthcare and social services.
• Maintain accurate documentation of interactions and outcomes.
• Collaborate with multidisciplinary teams to ensure comprehensive care.
• Provide crisis intervention and family education when needed.
• Monitor plan effectiveness and adjust as necessary.
