Job summary

Paycom - ATS seeks a dedicated Case Manager to support clients in a short-term life management program. You will interview applicants, develop intake and case plans, and maintain HMIS records while advocating for services.

The role requires two years of college coursework in Social Work and at least two years in social/public service settings. You will conduct progress meetings and coordinate life management classes to help clients cope with their environment.

Qualifications

• Interviews applicants to determine eligibility for program enrollment.
• Develops intake and case plans for client; instructs clients in completing paperwork and forms.
• Maintains case records and HMIS data entry and updates.

Responsibilities

• Meets regularly with clients to discuss progress and document records.
• Plans and facilitates life management classes for clients.
• Enforces house guidelines and disciplinary actions as needed.
• Maintains accurate and up-to-date client files and reports.
