Job summary

ADAPT Community Network in New York City is seeking a dedicated Case Manager to develop, implement, and monitor Life Plans for residents in our care. You will advocate for individuals, coordinate services, and engage families to promote dignity and independence in a supportive setting.

Under supervision of the Program Director, you will participate in interdisciplinary teams, document progress, and ensure compliance with regulatory requirements while fostering high-quality, person-centered care.

Qualifications

• Bachelor’s Degree in Human Services or related field.
• 1+ year of experience working with individuals with developmental disabilities.
• Ability to read, write, speak, and understand English.
• Strong interpersonal and communication skills with diverse stakeholders.
• Excellent organizational and time management abilities.
• Proficiency in Microsoft Office and electronic record systems.
• Ability to handle confidential information with discretion.
• Ability to work independently and in a team.

Responsibilities

• Develops and monitors Life Plans for individuals in residential settings.
• Coordinate services to align with regulatory standards and personal goals.
• Lead interdisciplinary team meetings and document recommendations.
• Collaborate with staff and providers for coordinated support.
• Maintain accurate documentation including monthly case notes and contacts.
• Provide supervision and guidance to Residence Program Specialists.
