Job summary

River Edge Behavioral Health Center is seeking a dedicated Core Case Manager to provide compassionate support and essential case management services to individuals with mental health conditions, substance use disorders, or developmental disabilities. You will connect clients to community resources, support recovery goals, and promote independence through skill-building and advocacy.

Qualifications

Bachelor’s degree in a social service-related field (e.g., Social Work, Psychology, Human Services).
Strong interpersonal and communication skills.
Knowledge of community resources and social services systems.
Organizational and documentation proficiency.

Responsibilities

Provide resource development and linkage support to individuals in home or community settings.
Monitor client progress toward individualized treatment goals.
Assist clients in accessing community resources (food, healthcare, financial aid, housing).
Deliver short-term case management and referrals for clients in emergencies.
Connect individuals to housing resources and support housing applications.
Offer skills training in independent living, coping strategies, and symptom management.
Transport clients using personal vehicles and make home visits.
