Job summary

LifeStream is seeking a Care Manager I to provide advocacy and support within the OPTUM program, linking clients to services and coordinating care to maximize independence. You will work closely with clients, families, staff, and community partners, attending staffing as needed and ensuring accurate documentation of services.

The role requires a Bachelor’s Degree in human services and at least two years in social services, with strong communication, organizational skills, and experience in case

Qualifications

• Bachelor’s Degree with major course work in the human services field required.
• Minimum of two years’ experience in the social services field required.
• Experience with chronically mentally ill in acute psychiatric or care management preferred.
• Ability to use case management software and EHRs effectively.

Responsibilities

• Links consumers to appropriate services and monitors progress.
• Collaborates with the individual, family, LifeStream staff and community partners to promote independence.
• Convenes/attends staffing to assess and coordinate services.
• Completes agency, funding, legal, statistical and fiscal documentation accurately.
• Direct care includes assessment interventions and case management.
• Regular driving between appointments with valid license is required.
• Other duties as assigned by Leadership.
