Qualifications

• Master’s degree in Social Work (MSW) preferred; Bachelor’s in social work, psychology, or related field required; equivalent experience may substitute.
• Minimum 5 years of case management experience with public and community-based organizations.
• Bilingual English/Spanish preferred but not required.
• Strong crisis and conflict management experience.
• Excellent analytical, oral and written communication skills.
• Proficiency in Microsoft Office and data management systems.

Responsibilities

• Provide high-quality service to clients with compassion and professionalism.
• Conduct intakes and assessments to determine client needs and eligibility for services.
• Develop individualized case plans supporting immediate and long-term goals.
• Support clients in reaching goals by identifying resources and removing barriers.
• Address emergency housing, income, health care, and transportation issues.
• Provide crisis intervention and short-to-intermediate counseling as needed.
• Maintain client data in HMIS/CORE and prepare various reports.
• Manage a caseload of approximately 20 clients with timely follow-through.
