Job summary

Camelot Community Care Inc. is seeking a qualified Child Welfare Case Manager to support children and families in our community. The role emphasizes collaboration with supervisors, community partners, and courts to ensure safety and permanency for children.

Qualifications

• Bachelor's degree in Social Work or related human services field.
• One year of relevant social services experience working with children and families.
• Maintain child welfare certification through ongoing training.

Responsibilities

• Participates in staff development opportunities and maintains child welfare certification through ongoing training.
• Backs up other team members and assists on cases.
• Maintains relationships with supervisory staff and community resource providers for continuity of care.
• Establishes relationships with children and families to promote permanency and safety.
• Completes case notes accurately and timely in the required data system.
• Prepares for and attends court activities, including hearings and testimony.
• Develops judicial reviews, child well being assessments, medical reviews, and required reports on time.
• Attends individual case staffings and conducts safety assessments.
• Makes referrals for services as needed and arranges emergency placements when necessary.
• Searches for and documents efforts to locate parents and family members.
• Completes home studies for prospective placements.
• Plans and facilitates visits between children in care and their families.
• Conducts minimum monthly face-to-face contacts with children and families.
• Reviews progress toward case plan goals and supports foster/adoptive families.
• Negotiates subsidy agreements with adoptive parents when applicable.
• Completes adoption paperwork leading to finalization as assigned.
• Meets adoption goals when applicable.
