Qualifications

• Bachelor's degree in social work or related field.
• Three years of progressively responsible experience with seniors or people with disabilities.
• High school diploma or GED with relevant education/experience.
• Must have a valid State of Oregon driver's license.
• Must pass a background check.
• Basic computer skills required.

Responsibilities

• Manage a caseload of adults with disabilities (ages 18–64) and senior citizens.
• Coordinate services and supports for clients in collaboration with internal teams.
• Work under the Unit Manager within Senior & Disability Services to ensure quality case management.
• Maintain accurate records and documentation consistent with program requirements.
