Job summary

Lane Council of Governments (LCOG) seeks a qualified professional to manage a caseload of adults with disabilities and senior citizens within the Senior & Disability Services division. You will handle new admissions and ongoing cases, coordinating with partners to ensure quality support and service delivery.

The role requires a Bachelor's degree in social work or related field, three years of relevant experience, basic computer skills, and a valid Oregon driver’s license.

Qualifications

• High school diploma or GED required.
• Bachelor's degree in social work or related field.
• Three years of progressively responsible experience with seniors or people with disabilities.
• Basic computer skills.
• Must have a valid State of Oregon driver’s license.
• Must pass a background check.

Responsibilities

• Manage a caseload of adults with disabilities (ages 18–64) and senior citizens.
• Handle newly admitted clients and ongoing cases.
• Coordinate with Senior & Disability Services (S&DS) and external partners.
• Maintain accurate client records and documentation in a timely manner.
