Accessible Space in Reno, NV is seeking a Full-Time Case Manager dedicated to supporting individuals in achieving goals and improving quality of life. You will lead admissions and orientation, coordinate interdisciplinary assessments, and guide treatment planning.

This role emphasizes collaboration with families, clinicians, and funding sources, ensuring accurate documentation and timely funding authorizations.

Qualifications

• Requires LSW credential or Master's in social work.
• Minimum of three years as a case manager in rehab or related setting.
• Experience in rehabilitation programs, day treatment, or residential programs for traumatic brain injuries.
• Strong communication and collaboration with multidisciplinary teams.

Responsibilities

• Conduct admissions and orientation, including tours, rights and responsibilities, schedules and expectations.
• Coordinate interdisciplinary assessments and support development and review of treatment plans.
• Lead conferences with patients, families, clinical staff and funding sources.
• Facilitate funding authorization, documenting communications with insurers and other sources.
• Lead clinical treatment planning sessions with the clinical team.
• Maintain accurate, confidential clinical documentation and track client progress.
• Provide updates and discharge planning support to families and caregivers.
